At Sakura Intermediate Care Facility, we understand that the decision to move a loved one to a long-term care community can have a significant impact on individuals and their families. Our social worker is here to not only help residents and families with the adjustment process, but also to provide continuing support throughout the stay, as physical, cognitive, and/or psychosocial needs may change over time.
We are committed to promoting a nurturing environment that helps each resident to thrive and achieve overall wellbeing. In this pursuit, we also strive to provide peace of mind to our families. Our social worker collaborates with the interdisciplinary team to facilitate care coordination and communication between residents, families, and staff. Our social worker offers additional services such as helping to develop a plan of care for the resident, advocating for residents and families, providing supportive services, connecting you to a wide range of community resources, and assisting with discharge planning.
